Attached is the medical records release form. You would use this same form for requesting medical records from another doctor to be sent to us as well as requesting us to send your medical records to another doctor. There is no charge for sending medical records to another doctor.
If you are needing medical records released directly to the patient, the charge is $25 for the first 25 pages and $.50 for each additional page. Please fax medical records release to 281-457-6409 and allow 48-72 hours. Please also include a good telephone number as someone will be calling you when medical records are ready for you to pick up.
